Once you schedule your first appointment, you'll receive an email with access to Vida Counseling's client portal. Please review and complete all forms prior to your appointment. This will ensure that we can maximize our clinical time. You'll also find the answers to the most common questions in the documents available in the portal and on our Q&A page.
Note:
For marital counseling, both partners need to fill out initial paperwork.
For minors whose parents are separated or divorced, both parents need to fill out initial paperwork.
